Step 2: Provide Basic Demographic Information
Fill out a brief form with your basic details, including:
Full Name
Date of Birth
Address
Phone Number
Email Address
Ensure all information is accurate, as this will help us provide the best possible service.
Step 3: Schedule Your Intake Specialist Appointment
Select a convenient time for an intake specialist to contact you. This appointment is crucial for setting up your program. You’ll receive a confirmation email with your scheduled time.
Fill out the red areas then pick a date that works for you. then hit search

Chose a time and hit next

Step 4: Complete the Intake Process
Once your appointment is scheduled you will receive 5 forms to complete inside your patient portal. They will not be all sent at once so please check back after filling out the first one.
Please log into your patient portal and complete the forms. Upload your valid state-issued ID (This must be done before you speak to the intake specialist) If you don’t get the forms or have any issues, reach out to us we will help you through it
